Clinical Features and Treatment Modalities of Ectopic Pregnancy

|
Objective: Even though ectopic pregnancy is still cause of maternal death, the mortality rate of ectopic pregnancy was significantly decreased. So early diagnosis and proper management is very important. We present clinical features and the result of treatment modalities for precise diagnosis and management.
Methods: Using a retrospective study, 121 women who were diagnosed ectopic pregnancy at Department of Obstetrics and Gynecology, Konyang university hospital between Feb. 1, 2000 and Aug. 31, 2003 were included. Data were described and analized by Paired Sample T-test.
Results: The incidence of ectopic pregnancy was 1 in 24.3 deliveries. The most common age group was 30-34 years group. A past history of abortion was in 66.9%. The most frequent intervals between last menstrual period and the onset of symptom was 6-8 weeks in 45.5%. Lower abdominal pain was most common clinical menifestation, which was developed in 71.1%. Ectopic mass was implanted on fallopian tube in 93.3%, especially ampullar portion in 81.9%. Total amount of intra-abdominal blood loss between 100-499 ml was in 40.6%, above 2000 ml in 5.9%. Of total 121 cases, operative procedures were done in 105 cases (explo-laparotomy in 58 cases, laparoscopy in 43 cases, dilatation & curettage in 4 cases) and medical treatment (Methotrexate treatment) in 16 cases. Of operative procedures, salpingectomy was in 63.8%, salpingostomy in 12.4%, salpingotomy in 8.6%. Among 21 cases initially treated by medical treatment, 16 cases were successful and the success rate was in 72.6%. According to operative treatment modality, there was significantly difference in serum ¥â-hCG level between pretreatment and posttreatment.
Conclusion: Based on these results, it is very important that we should have an interest in ectopic pregnancy for early detection and proper management. Precise decision and successful treatment is possible to decrease mortality and increase fertility. It may contribute to further improvement of treatment in ectopic pregnancy.
